You can either fax the forms back to us at (305)933-4438, email them to medassist@pediendomiami.com, or bring them with you to your appointment. In this way we can request medical records from your child's doctor prior to your visit and you will save time on the day of your appointment.

How To Find Us
We are located in downtown Aventura, just south of the Aventura Mall and the William Lehman Causeway.

From the north, take I-95 to exit 16 (Ives Dairy Rd). Go east to Aventura. Turn right (south) on US-1 (Biscayne Blvd), then turn left on 191st Street.
From the south, take I-95 to exit 14 (Miami Gardens Dr). Go east to Aventura. Turn left (north) on US-1 (Biscayne Blvd), then turn right on 191st Street.
Look for the Concorde Centre II building entrance on your left, in the corner where 191st Street turns sharply to the right.
